Who do you support?

We support anyone registered with the following GP surgeries:  Burnley Group Practice, Padiham Medical Centre, Rosegrove Surgery, Ightenhill Medical Centre and Riverside Family Practice.

If you are not registered with one of these surgeries, and would like to be, we can also support you to register.

How do I get referred in for support?

You can be referred in via your GP surgery (you can ask to be referred to Burnley West Health and Wellbeing Team), or by any professional you are being supported by.  You are also welcome to self-refer yourself in.  You can do this by emailing our team on bwpcn@nhs.net

Once we receive your referral or query we aim to be in contact with you as soon as possible.

What kind of support can I receive from your team?

We have a wide range of professionals in the team who can support you to manage various worries and concerns.  It all depends on what matters to you at this time.  We can help you look at your needs and priorities and help you achieve meaningful changes which will positively affect your wellbeing and day to day life.  During our first conversation we will get a clear picture of what life looks like for you right now and will help you create a plan which feels good for you.
